1: Go to the myBayCare Login Page

2. Click on “Forgot Username?”

• Beneath the login fields, click the link that says “Forgot Username?” or copy my.baycare.net/BaycareClinicsMyChart/recoverlogin.asp

3. Enter Your Account Details

• Provide the following information to verify your identity:
  • First and last name
  • Date of birth
  • The email address is associated with your myBayCare account.

4. Submit the Form

• After entering the required information, click “Submit” or “Continue”.
